You will be asked to fill out some forms specific to the hormone program or the TBI program. After your forms are completed, a member of our clinical team will review your enrollment package to determine if you are a candidate for our programs. If you are accepted, you must pay the enrollment fee for the first consultation before you can start the program and any lab requisitions are created.

Step 2: Labs/Bloodwork
To clearly understand what is affecting you, we require extensive laboratory testing involving bloodwork on all of our patients. A lab requisition with instructions will be sent for you to complete your bloodwork.
Step 3: evaluation/consultation
After you have completed your bloodwork and your lab results are reviewed, you will be scheduled for your initial consult appointment with Cheryl to review and discuss your results and treatment plan.
